Advanced Practice Nurse

Provides primary or specialty health care services within defined scope of practice in collaboration with a licensed physician and in accordance with MCW//CHW policies and procedures.

Department Position Purpose:
Responsible for overseeing the clinical management and continuity of care of pediatric epilepsy and general neurology patients and families as an active member of the Comprehensive Epilepsy Program. Rotational responsibilities across inpatient and outpatient settings.

Physician Assistant

Provides primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/CHW policies and procedures.

CORE RESPONSIBILITIES

1. Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.

2. Orders and interprets laboratory and diagnostic tests.

3. Develops pharmaceutical and non-pharmaceutical treatment plans.

4. Monitors effectiveness of plan of care.

5. Educates patients on disease and treatment plans.

6. Performs procedures as trained and appropriately credentialed.

7. Documents within the Electronic Medical Record (EMR) and completes other necessary documents.

8. Collaborates and/or consults with physicians and other health care providers as appropriate.

9. Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.

Department Specific Responsibilities:
1. Coordination of Care- Inpatient Setting:
a. Admits and coordinates care for elective and urgent patient admissions to the EMU.

b. Performs history and physical exams; writes daily pediatric neurology progress notes and all written documentation required for discharge.

c. Offers input on discharge needs; evaluates effectiveness of discharge planning as a transition to home and adaptation to illness for a patient population; assesses and negotiates systems to maximize use of current resources or creates new community resources.

d. Orders, performs and interprets screening and diagnostic procedures and tests, and appropriately communicates and documents information.

e. Takes primary responsibility for effective interdisciplinary coordination of care for all patients in the EMU.

2. Coordination of Care- Ambulatory Setting:
a. In collaboration with physician colleague(s), assumes primary responsibility for a selected caseload of patients in the ambulatory setting.

b. Performs history and physical exams; obtains a comprehensive health, developmental, and family history on pediatric and adolescent patients.

c. Dictates pediatric neurology progress notes and completes all written documentation required for patients seen in the Neurology Clinic.

d. Orders, performs and interprets screening and diagnostic procedures and tests, and appropriately communicates and documents information.

e. Collaborates effectively with other members of the health care disciplines to provide comprehensive care to patients. Documents the plan in order to effectively facilitate follow-through and outcome evaluation.

f. Evaluates responses to interventions and modifies the plan of care for patients under his/her care.

g. Collaborates with home care/community agencies to assume continuity of care for assigned caseload.

MINIMUM POSITION QUALIFICATIONS

Education Requirements

Advanced Practice Nurse

Master’s Degree in Nursing â€" required.

Physician Assistant

Bachelor’s Degree or higher in Physician Assistant Studies

Experience

Nurse Practitioner:
Specialty pediatric nursing experience â€" required.

Pediatric /Neuroscience nurse practitioner experience highly preferred.

Physician Assistant:
Pediatric /Neuroscience nurse practitioner experience highly preferred.
