Genetics Coordinator/Counselor

POSITION SUMMARY: The Genetics Coordinator/Counselor is responsible for participation in the development, establishment, coordination and provision of Genetic Counseling services. Responsibilities include but are not limited to: program development, development and implementation genetic screening protocols; serving as a expert resource to staff including practitioners and Care Management staff; community outreach; and the provision of genetic counseling for diagnosed genetic conditions, congenital anomalies, or family history of known or suspected genetic disorders.

The Genetics Coordinator/Counselor serves under the general guidance, direction and supervision of the Clinical Health Education Manager. Additionally, the incumbent works in conjunction with and collaborates with internal and external providers/specialists, organizations and insurance staff related to genetics services.

JOB SPECIFIC QUALIFICATIONS

1. Master's Degree from an American Board of Genetic Counseling accredited training program in Human Genetics or Genetics Counseling. 2. Board certification by the American Board of Genetic Counseling (ABGC) or American Board of Medical Genetics (ABMG). 3. Minimum of three (3) to five (5) years of experience in Human Genetics Counseling. Experience in prenatal/pediatric counseling is preferred. Leadership experience is highly desirable. 4. Possession of a valid, Wisconsin driver license with a good driving record; or ability to travel to various work sites. Employee is expected to provide personal transportation. 5. Aptitude in basic sciences, especially in the area of genetics. Knowledge of human genetics, testing protocols and test interpretation. Ability to maintain currency in area of expertise in human genetic including new, emerging and state of the art technologies, testing and other initiatives in medical and counseling genetics. 6. Ability to perform patient assessment and design individualized genetics related patient action plans. Ability to coordinate patient genetics related care and follow-up required. 7. Knowledge of counseling skills and techniques and ability to apply especially with sensitivity in the area of genetics. 8. Knowledge, skill and ability in writing genetic /counseling reports and interpreting test results. Ability to provide consultation, determine testing and read and interpret reports various medical reports and/or results, and advice concerning genetic testing across the organization and to GHC-SCW members. 9. Ability to learn and maintain currency with GHC-SCW, UW Health, St. Mary’s /Dean policies and other partnerships procedures as applicable to genetics testing and counseling. 10. Knowledge of and ability to effectively use human relations, interpersonal and communications skills. Excellent interpersonal skills required. Excellent verbal and written communication skills, including active listening skills. 11. Ability to provide culturally competent services and to work effectively with persons of diverse cultural backgrounds. 12. Knowledge of medical records and appropriate documentation requirements, and knowledge of HIPAA requirements, required 16. ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Establish a Genetics Program at GHC-SCW. Initiate, develop and implement genetic screening protocols in order to enhance clinical services offered to members. 35 % Improve patient access to prenatal genetic services. Services would include but are not limited to:

A. Set up 1st Trimester Screening Program at GHC-SCW. Offer members genetic counseling services in conjunction with the initial OB visit. B. Prenatal genetic counseling as appropriate.

  • Work closely with OB Coordinator and OBs to determine appropriate timing of genetics visit and content of our discussions to minimize overlap of information.
  • Explore options of non-traditional genetics counseling such as: Telemedicine (relaying information by phone).
C. Offer prenatal screening classes (e.g. offering monthly education seminars to review general information on prenatal screening and diagnostic testing options in a group setting; D. Offer genetic counseling for fetal ultrasound findings. E. Offer updated clinical services such as Nasal Bone assessments as appropriate and rapid turnaround times for results. F. Develop corporate accounts with designated testing laboratories and work with GHC-SCW Radiology Department to obtain nuchal translucency accreditation. G. Develop a full set of protocols, materials and training for providers related to 1st Trimester Screening. H. Create synergy between screening program and genetic counseling.

  • Include discussion of ancestry based carrier screening at initial genetics visit. Discuss potential benefits and limitations of screening.
  • Determine appropriate laboratory tests according to ethnicity and family history.
o Caucasians (Cystic Fibrosis). o Asians/Mediterranean descent (Thalassemia). o African American (Hemoglobinopathies). o Ashkenazi Jewish (Jewish disease panel). o French Canadian/Cajun (Tay-Sachs).

  • Develop informed consent procedures and materials
  • Provide follow-up genetic counseling for patients with positive carrier screens.
o Review information on disease and implication of positive carrier status. o Provide risk estimates regarding the likelihood of the disease in the current pregnancy. o Discuss benefits, risks and limitations of carrier testing for partner/ father of pregnancy. o Discuss overlap with WI Newborn Screening program. o Review implications for other family members.

o Assist providers/laboratory with ordering appropriate tests.

o Explore option of offering carrier testing panels.

2. Provide genetic counseling for known genetic conditions, congenital anomalies, or family history of known or suspected genetic disorders. 40%

A. Obtain and review medical and family histories, document family history and summarize pertinent information. B. Summarize pertinent data from published literature, databases, internet and other professional sources.
  • Risk Assessment- Assess the risk for occurrence or recurrence of a genetic condition or birth defect.
  • Counseling- Explain to patients the diagnosis or potential occurrence of a genetic condition or birth defect.
  • Discuss available options and delineate the risks, benefits and limitations of appropriate tests and clinical assessments.
  • Coordinating genetic testing/family studies.
C. Determine the appropriateness of genetic testing based on the relevant patient/family history and medical literature. D. Obtain/document informed consent for genetic tests or related procedures. E. Interpret genetic test results in light of available clinical data and known limitations of testing techniques. F. Communicate results of genetic tests to patients and referring providers. Guidance regarding medical management for genetically related conditions. G. Determine the patient's need for additional testing, referral or assessment. H. Inform patients of management recommendations, including implications for at-risk family members. I. Inform providers of management recommendations, utilizing office visit summary notes, phone contact, and/or provision of relevant literature. Genetic Case management. J. Assist patients in scheduling specialty consults and/or additional testing. K. Assist patients in evaluating the risks, benefits and limitations of participation in research, and facilitate the informed consent process and sample collection.

3. Serve as the internal expert in Genetics for GHC-SCW clinics and the other partner primary care delivery systems. 10% A. Triage/manage genetics referrals within the system.
  • Assist in determining the necessity of genetics counseling and appropriate timing/urgency of appointments.
o Review patient's medical records and use clinical judgment regarding the indication for referral and facilitate scheduling accordingly. o Contact patients directly to obtain additional information as needed (e.g. obtaining family history information, collecting outside records).
  • Assist providers in determining necessity of referrals to outside clinics (e.g. maternal-fetal medicine or medical genetics clinics).
B. Address calls and emails from providers and clinical staff requesting information regarding genetics indications (e.g. abnormal screening results or inherited conditions) and provide guidance regarding appropriateness of genetic services or clinical management recommendations. C. Assist GHC-SCW laboratories in facilitating genetic testing.
  • Determine preferred laboratories for genetic testing by researching available laboratories and comparing factors such as cost, detection rates, turnaround times, etc.
o When possible, establish institutional billing accounts for reduced fees/negotiated pricing.
  • Assist providers in coordinating genetic testing for patients seen by outside genetics providers (e.g. geneticists at UW-Waisman) so that testing is billed through GHC-SCW laboratory rather than outside laboratories.
  • Establish a protocol to assist GHC-SCW laboratories with logistical issues regarding genetic testing, e.g. completing necessary paperwork (clinical history information, test requisition forms, consent forms) and specimen packaging and shipping.
4. Serve as the expert in Genetics for GHC-SCW insurance. 10%

A. Assist GHC-SCW Care Management department staff in determining the appropriateness of requests for outside genetic services (e.g. genetic testing or medical genetics evaluations). B. Provide assistance with development of health plan policies requiring genetic expertise. C. Develop/revise a formal genetic testing policy for GHC-SCW. D. Develop system-wide recommendations/genetic testing protocols for certain indications (e.g. genetics work-up for mental retardation, developmental delay, and autism diagnoses).

5. Community Outreach. 5%

A. Provide ongoing genetics education for medical professionals at department meetings, or CME programs. B. Provide education/support to patient populations, including: Developing educational material pamphlets; establishing support groups, as needed. C. Offer job shadowing opportunities for genetic counseling students, as approved.

6. Perform other duties as required/requested.
